Family Preservation Community Services (FPCS) of Orange County, California surrounds referred children, youth and their families in a network of care that will do “whatever it takes” to meet the needs of the family so that the child or youth may remain with his or her birth family, relative caretaker or foster family. By working with multidisciplinary teams, children and their families will be linked to support systems that will build upon child/family/community strengths to improve child and family functioning.
FPCS will link each referred child and family with a Wraparound Family Team which will consist of a Care Coordinator whose primary role is to coordinate the wraparound process and to ensure objectives are accomplished and needs are met. Other team members include a Parent Partner, a Youth Partner (when age appropriate), the child’s parents or caretakers and other invited members from the family, community, service providers and other agencies. The Wraparound model used by FPCS has been designed by the National Wraparound Initiative and will be evaluated, in part, by using the Wraparound Fidelity Index and other internal evaluation tools. Services may include, but not be limited to, engagement, assessment, service planning, intensive case management, 24/7 access to services, crisis prevention and intervention, community referrals and linkage, benefits acquisition, transportation assistance, mentoring, skill building activities, life skills training, respite care, transition planning and aftercare services.
Our Wraparound program is highlighted by the following characteristics:
Community-Based: All services and supports will be provided in the family’s community, often in the child’s home, school and other lest restrictive environments. Restrictive care is reserved for brief periods of stabilization only when all other resources have been exhausted.
